Eating well is crucial for maintaining good health and for disease prevention . At Centerpoint Health, our Registered Dietitian, Darieli Sanchez, can assist you with health conditions such as diabetes, eating disorders, cardiovascular disease, kidney disease, obesity and much more by creating personalized plans to improve your health.
If you're interested in improving your nutrition and you're a patient at Centerpoint Health, give us a call to schedule an appointment with our Registered Dietitian. If you're not a Centerpoint Health patient, we can still assist you. One of our primary care doctors will conduct a medical evaluation and then refer you to our Registered Dietitian.
What to expect at your first appointment: During this initial meeting, our nutrition expert will learn about your health history and dietary needs. Then, they will work with you to create a suitable plan and set goals based on your individual health needs. Subsequent appointments will be scheduled to monitor your progress and make necessary adjustments to your plan.

Centerpoint Norwood School-Based Health Center
Centerpoint Health's school-based health center (SBHC) in Norwood, OH, provides a range of services to meet the healthcare needs of the community. If you or your child already have a healthcare provider, we will work with that provider to offer consistent care.
What we offer:

What if my student has a doctor? Not a problem! Centerpoint Health will work collaboratively to make sure the healthcare needed by your students is addressed and shared with that office.

Title X Family Planning Services
At Centerpoint Health, your well-being and family planning needs are our top priority. We are committed to delivering comprehensive healthcare, including Title X family planning services, available to all individuals irrespective of their ability to pay. Title X services are voluntary, ensuring confidentiality without being a prerequisite for any other services.
What is Title X?
Title X is a federal program administered by the U.S. Department of Health and Human Services, Office of Population Affairs (OPA). Its primary goal is to ensure access to family planning and reproductive health services for everyone, regardless of income or insurance status. Centerpoint Health actively participates in this program, proud to provide quality care to our community.
Title X offers affordable or no-cost reproductive health care services for both women and men, including birth control, pap smears, breast exams, STI testing, and more. The program focuses on serving individuals and families with low incomes, operating through grants to over 3500 clinical sites, including public health departments and non-profit health centers.
For those with family incomes below 100% of the most recent federal poverty level (FPL) guidelines, Title X services are provided at no charge. Additionally, services are discounted on a sliding scale for individuals with family incomes between 101-250% of the FPL. In 2019, 64% of clients had family incomes at or below 100% FPL, while 88% had family incomes below 250% of the FPL.

More about Title X – The Importance and the Need
- Publicly funded family planning clinics help prevent 1.94 million unintended pregnancies.
- Six in 10 women consider publicly funded family planning clinics their usual source of medical care.
- Title X projects may also include other reproductive health and related preventive health services such as HPV vaccination, provision of HIV pre-exposure prophylaxis (PrEP), and screening for obesity, smoking, drug and alcohol use, and mental health.

Our office has a full-service, state-of-the-art dental facility which offers the following services:

Safe Care, Safe Space Dental
Your primary care for pediatrics can include all of the following:
· Routine wellness visits and evaluation of your child’s medical, developmental, and social health
· Urgent sick visits and treatment of acute illness
· Management of chronic health problems such as asthma, allergies, and diabetes
· Immunizations
· Hearing and Vision Screenings
· Sports Physicals
· Referrals to specialists when needed
Centerpoint Health offers vaccines!
We have Influenza vaccines and TDaP available for all Centerpoint Health Patients.
We offer routine childhood immunizations through Vaccines for Children (VFC) for those children that qualify.
We do our best to support all children from various family situations including children in foster care.

Insurance Enrollment
Centerpoint offers Outreach and Enrollment services to help you. We offer FREE insurance enrollment by Certified Application Counselors for those who do not currently have health/dental insurance.
Our specialists are also available to assist you in signing up for Medicaid and with the Marketplace. We are available for open enrollment and to assist throughout the year due to a qualifying event, such as getting married, having a baby, or moving. Patient-Centered Medical Home
Centerpoint is certified as a "Level 3" 2022-2023 Patient-Centered Medical Home at all medical locations. We were the 3rd Federally-Qualified Health Center (FQHC) to receive this designation in Ohio.
What is a Patient-Centered Medical Home (PCMH)?
A "medical home" is a medical office or clinic where a team of health professionals work together to provide a new, expanded type of care to patients. The goal is to build on the relationship you already have with your provider to ensure you receive the care you deserve and need.
