Eating well is crucial for maintaining good health and for disease prevention . At Centerpoint Health, our Registered Dietitian, Darieli Sanchez, can assist you with health conditions such as diabetes, eating disorders, cardiovascular disease, kidney disease, obesity and much more by creating personalized plans to improve your health.

If you're interested in improving your nutrition and you're a patient at Centerpoint Health, give us a call to schedule an appointment with our Registered Dietitian. Family Planning Services

Title X Family Planning Services

At Centerpoint Health, your well-being and family planning needs are our top priority. We are committed to delivering comprehensive healthcare, including Title X family planning services, available to all individuals irrespective of their ability to pay. Title X services are voluntary, ensuring confidentiality without being a prerequisite for any other services.

What is Title X?

Title X is a federal program administered by the U.S. Department of Health and Human Services, Office of Population Affairs (OPA). Its primary goal is to ensure access to family planning and reproductive health services for everyone, regardless of income or insurance status. Centerpoint Health actively participates in this program, proud to provide quality care to our community.

Title X offers affordable or no-cost reproductive health care services for both women and men, including birth control, pap smears, breast exams, STI testing, and more. The program focuses on serving individuals and families with low incomes, operating through grants to over 3500 clinical sites, including public health departments and non-profit health centers.

For those with family incomes below 100% of the most recent federal poverty level (FPL) guidelines, Title X services are provided at no charge. Additionally, services are discounted on a sliding scale for individuals with family incomes between 101-250% of the FPL. In 2019, 64% of clients had family incomes at or below 100% FPL, while 88% had family incomes below 250% of the FPL.

Our Title X Services

  • Confidential Counseling: Experienced healthcare professionals offer personalized guidance for your family planning concerns with the utmost confidentiality.
  • Birth Control Options: Choose from a range of birth control methods with our team guiding you through informed choices.
  • Preventive Screenings: Regular screenings for STIs and preventive measures ensure your overall health.
  • Reproductive Health Education: Comprehensive education empowers you with the information needed to take charge of your well-being.

Why Choose Centerpoint Health for Title X Services?

Compassionate Care: Our team is dedicated to providing compassionate and non-judgmental care, ensuring your support throughout your healthcare journey.

Accessible Services: We believe in removing barriers to healthcare access, offering affordable and quality care designed to be accessible to all.

Respect for Your Choices: Your choices regarding family planning are deeply respected at Centerpoint Health, supporting you in every decision for your reproductive health.

Schedule your appointment! Take a proactive step towards prioritizing your reproductive health. Your well-being is our priority.

 Patient-Centered Medical Home logo

Patient-Centered Medical Home

Centerpoint is certified as a "Level 3" 2022-2023 Patient-Centered Medical Home at all medical locations.  We were the 3rd Federally-Qualified Health Center (FQHC) to receive this designation in Ohio.

What is a Patient-Centered Medical Home (PCMH)?

A "medical home" is a medical office or clinic where a team of health professionals work together to provide a new, expanded type of care to patients.  The goal is to build on the relationship you already have with your provider to ensure you receive the care you deserve and need.

How does this work for you?

We are here for you and your health in several ways:

  • we are available 24/7;
  • we know you and remember your health history;
  • we take the time to ensure you understand your condition(s) and how to take care of yourself;
  • we help you coordinate your health care with specialists, behavioral health services, get appointments, and make sure specialists have all the information they need;
  • we help you transfer records from other providers, when needed;
  • we work with you to help set goals for your care and help you reach those goals one step at a time.
