Patient Survey Highlights from 2022:
- 100% of dental patients surveyed said their dentist at Centerpoint was friendly, helpful, and took time to answer all of their questions.
- 95% of medical patients surveyed noted that their provider was friendly and helpful, listened to them, answered their questions, and spent enough time with them during the visit.
- 96% of our Behavioral Health patients found it easy to get an appointment and liked our hours of operation. 100% thought our providers were respectful, helpful, and helped them meet their treatment goals.

Quality Highlights from 2022:
- Diabetes Management - 71% of our diabetic patients have an A1C less than 9.0
- Blood Pressure - 68% of our patients have blood pressure controlled at less than 140/90. The Healthy People 2020 goal is 61.2% . (Healthy People HDS-12)
Centerpoint Health cares about your chronic condition. We have a Care Management program to help you make progress with your care plan goals, give you an advocate for needed services and health education, and help decrease avoidable emergency room visits and hospitalizations. We educate, monitor, and evaluate our patients with chronic conditions, such as diabetes and hypertension, on a regular basis. We believe a proactive approach helps Centerpoint patients reach their overall healthcare goals, and reaching those goals leads to healthier and happier lives.

MEDICATION-ASSISTED TREATMENT WITH THE BEST CHANCE AT RECOVERY
AT CENTERPOINT HEALTH
Middletown, Ohio | May 21, 2019… Persons thinking about cutting down on alcohol or opioid use now have a new team at Centerpoint Health in Middletown and Franklin.
Centerpoint Health provides primary medical care, dental, pediatric, maternal and gynecological care. They are now adding a Vivitrol Medication Assisted Treatment program.
“At Centerpoint Health we have a great commitment to our community. That is why every time we seek to expand our services to cover all patient needs. We are now offering a Substance use Treatment program to current patients and new patients who make Centerpoint Health their primary care provider,” said Lorie Glenn, Chief Executive Officer of Centerpoint Health.
Our program consist in a medication-assisted treatment with Vivitrol that offer the best chance at recovery. Vivitrol is a monthly shot which reduces cravings and blocks ‘the high’ of either opioids or alcohol. Vivitrol blocks opioid receptors in the brain for one month at a time, helping patients to prevent relapse to opioid dependence, following detox, while they focus on counseling,” explained Lorie Glenn, Chief Executive Officer of Centerpoint Health.
The residents of the Southwest Ohio area looking for a ‘Vivitrol doctor’ need look no farther than the primary care offices of Centerpoint Health. Their substance use counselor Patrick calls his office the ‘no judgement zone’. He is happy to talk with anyone questioning their alcohol or other drug use.
Centerpoint Health’s offices are at 231 N. Breiel Blvd., Middletown and 333 Conover Drive in Franklin. Appointments can be made at either office at 513 318-1188.

Our History
Centerpoint Health was created through a collective effort to improve health in the region. This effort was led by a large group of caring individuals representing the residents of Warren County. Participants included the healthcare community, social service organizations, local government, schools, and private businesses who wanted to make it easier for people to access high quality, affordable healthcare locally.
2023
- New Administrative Office opened in Franklin, OH.
2022
- The Centerpoint Norwood School-Based Health Center opens its doors to the community at-large.
- Centerpoint opens a pharmacy at its Middletown location, providing convenient and economic service.
2021
- Centerpoint Norwood School-Based Health Center opens in Norwood, Ohio, for students and staff of Norwood City Schools.
- Centerpoint is recognized by HRSA as being in the top 20% of community health centers nationwide (silver quality) and as a center which is advancing health outcomes through technology.
2020
- Centerpoint begins offering Medication-Assisted Treatment (MAT).
2019
- Centerpoint implements the Team Care Medicine (TCM) model of care.
2018
- Centerpoint starts OB/GYN services in Middletown through a collaboration with UC West Chester.
2017
- Centerpoint opens a new site in Middletown, Ohio.
- Centerpoint begins providing psychiatry services in Middletown and Franklin.
- Centerpoint expands the Franklin location by another 2000 square feet.
2016
- Centerpoint receives a dental expansion grant and obtains federal malpractice insurance (Federal Tort Claims Act).
- Centerpoint receives a second quality award from HRSA.
- Centerpoint achieves Level 3 Patient Centered Medical Home (PCMH) accreditation.
2015
- Centerpoint receives its first quality award with HRSA.
2014
- Centerpoint Health achieves Federally Qualified Health Center (FQHC) status.
- Centerpoint Health opens its doors at 333 Conover Drive, Suites B & D, providing sliding-fee services for medical, dental, and behavioral health to the Franklin-Carlisle area in Warren County on February 24, 2014.
2013
- The Health Resources and Service Administration (HRSA) of the Department of Health and Human Services grant is awarded for the new access point in Franklin.
- Bylaws, Strategic plan and goals, and a Quality plan are approved.
- The Conover Drive location in Franklin is chosen.
- The Centerpoint Health name is chosen, and a trade name registration is filed.
2012
- A Planning Grant is awarded to Talbert House to research the need for a new access point for medical services for the uninsured, unserved, and underserved population in Franklin-Carlisle in Warren County.
- An Area Needs Assessment of Franklin-Carlisle area is conducted by Wright State University.

2022 Quality Awards
As a Federally Qualified Health Center (FQHC) which receives funding based on our achievements, Centerpoint Health reports on our performance to the Health Resources and Services Administration (HRSA), part of the United States Department of Health and Human Services.
Each year, we report on the successes and challenges of our programs by providing clinical data on patient demographics and services. We illustrate our achievements by reporting costs, revenues, and more. The results and our scores are directly related to the funding we receive from HRSA; that funding is then reinvested in quality improvement projects to enhance the healthcare services we provide to our patients.
These awards are vitally important – with them we receive funds that help us continue to provide exceptional patient care.
Based on our data from 2021, Centerpoint Health was awarded:
Access Enhancer, recognizing Centerpoint Health has increased the total number of patients and the number of patients who receive at least one comprehensive service (mental health, substance abuse, vision, dental, and/or enabling) by at least 5% during consecutive reporting periods (2020 and 2021 UDS reporting years).
Advancing Health Information Technology (HIT) for Quality, an award which recognizes health centers using technology to increase access to care and advance quality of care. We're working hard to make sure you can easily receive healthcare and monitor chronic conditions from the comfort of your home using the latest technology. For example, Centerpoint Health offers blood-pressure cuffs which transmit information directly to your provider.
Patient-Centered Medical Home Recognition, a recognition which shows Centerpoint Health provides care which is patient-centered, comprehensive, team-based, coordinated, accessible, and focused on quality and safety. Centerpoint Health celebrated its fifth consecutive year as a patient-centered medical home in 2022!

Health Center Quality Leader - Centerpoint Health was recognized as being in the top 20% in overall clinical quality measure (CQM) performance among all health centers nationwide.

Good Faith Estimate
You have the right to receive a "Good Faith Estimate" explaining how much your medical care will cost.
Under the No Surprises Act, as of January 1, 2022, health care providers need to give an estimate of the bill for medical items and services to patients who don’t have insurance or who are not using insurance.
Subsequently:
- You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.
- Make sure your health care provider gives you a Good Faith Estimate in writing at least one business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.
- If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
- Make sure to save a copy or picture of your Good Faith Estimate.

Kent Youngman brings over 20 years of CEO experience, including seven years leading another Federally Qualified Health Center (FQHC) in Ohio. He has a strong understanding of Centerpoint Health’s mission, values, and goals. Currently, Kent works closely with the senior leadership team and Board to ensure the continuity of strategic initiatives while fostering a positive and collaborative work environment. With his expertise, leadership, and compassion, he is committed to guiding Centerpoint Health through this important chapter and supporting the organization’s ongoing success.

Centerpoint Health impacts more than five thousand patients per year
and continues to grow
Middletown, Ohio | March 2019 Centerpoint Health is celebrating its 5th anniversary. What began as a small company has risen to become a leading service provider in healthcare to the community.
Centerpoint Health officially opened its doors on February 24, 2014 as a Federally Qualified Health Center. Our mission is to engage patients in high quality and culturally sensitive health services. We also offer affordable primary care services to Southwest Ohio, focusing on the medically underserved, unserved, and uninsured.
“We provide affordable, high quality, accessible primary care, dental care, psychiatry, obstetrics and gynecology care and behavioral health services to the community. Our goal is to improve the health outcomes and standard of livings of the communities as a whole,” says Lorie Glenn, Chief Executive Officer of Centerpoint Health. Centerpoint will be offering Alternative Therapy Services soon and Medication Assistant Treatment services in March.
Centerpoint Health started this community effort with a location in Franklin, then expanded to Middletown. In the beginning they had 11 employees, now they have more than 50 employees and serve thousands of patients per year.
“Our growth has been steady along with our expansion of services. We continue with firm steps contributing to our community” explained Ms. Glenn. We appreciate being part of building a healthier community.

Jennifer N. Feldman, M.D. is the Chief Medical Officer at Centerpoint Health. She is a board certified general pediatrician. She was raised in Loveland, OH, and is a graduate of the University of South Carolina and the Wright State University Boonshoft School of Medicine. She completed residency in Memphis, TN, at LeBonheur Children's Hospital through the University of Tennessee Health Science Center. Before joining Centerpoint Health, Dr. Feldman worked as a pediatrician at a Federally Qualified Health Center (FQHC) in Cincinnati, OH.

Director of Clinical Operations Mitzi Spurlock joined Centerpoint in 2018 as the LPN Team Lead with 19 years of experience in a clinical setting, and she was promoted to Director of Clinical Operations in 2021 after becoming a registered nurse. She is currently working on obtaining her bachelor’s degree through the Denver College of Nursing. Mitzi loves camping and traveling with her husband and children.
